Confidentiality & Access to Information
Everyone working for the NHS has a legal duty to keep information confidential. We ask you for information about yourself so that you can receive the correct care and treatment. We keep this information, together with details of your care because we need it if we see you again. You may be receiving care from a number of different NHS professionals and for your benefit we may need to share information from time to time. We will only pass on information to other professionals when there is a genuine need and it is in your interests. Anyone who receives information is under a legal obligation to keep it confidential.
You can request to view or receive a copy of your medical records in accordance with the Data Protection Act 1998. All requests must be put in writing to the Health Centre Manager. There will be a charge for this service in line with current BMA rates.
Chaperone
There are occasions when patients need to be examined by a Doctor that may involve an intimate examination. If you wish to have a chaperone present, please do not hesitate to ask the Doctor. It may not be possible for a chaperone to be provided immediately and you may have to return for your examination to be carried out at a mutually convenient time.
Non-NHS Forms & Information
A fee is applied, within the current BMA guidelines, for special forms or examinations that may be required which is not part of the NHS treatment scheme. Fees
BALANCE STREET PRACTICE PRACTICE FEES (2008/2009)
Fees are applied to Non-NHS Services and certain ones of these services are subject to VAT following new legislation, which came into effect from 1st May 2007.

Suggestions & Complaints
Balance Street Health Centre makes every effort to form excellent relationships with our patients and we endeavour to provide a friendly and helpful service to all our patients.
If you have any helpful suggestions of how we can improve our services, or would like to let us know of things we are doing particularly well, please inform the Health Centre Manager, either in person or in writing.
However, if you are dissatisfied with the service you have received by the Balance Street Health Centre or a member of our practice team, please address your complaint in writing to the Health Centre Manager.
Balance Street Health Centre has a written complaints procedure which is available for inspection if required.
